Position Summary
Provides Physical Therapy services to patients in their homes, assessing functional levels and developing treatment programs to improve independence and physical/cognitive functioning. This role includes patient and caregiver education, supervision of support staff, and ongoing progress reporting.
Qualifications Required:
Education: Master's degree or higher
Experience: Healthcare Setting (1+ years)
Licenses/Certifications: Valid Physical Therapist license in the state of Illinois (IL) Valid Regular Driver's License (DL)
Preferred: PT license number (to be provided during application)
Overview
Schedule
Monday – Friday, 8: 00a – 5:00p Plus one weekend per month and 2 holidays per year Typically sees 5–6 patients per day
Travel: Local travel to residential homes required
Key Responsibilities
Perform Physical Therapy assessments and develop individualized treatment programs
Record and report patient's reaction to treatment and changes in condition
Instruct patients and caregivers in therapy programs and proper device use
Supervise physical therapy assistants and, when appropriate, certified nursing assistants
Prepare reports and participate in staff and Agency development activities
