Home Health Care | Full-Time
Position Summary
Provides in-home Occupational Therapy services to Agency patients as prescribed by the attending physician, and offers Occupational Therapy consultation to Agency staff and physicians as needed.
Qualifications Required:
Education: Bachelor's degree or higher
Experience: Occupational Medicine (1+ years) Social Work (2+ years)
Licenses/Certifications
Valid Occupational Therapist, Registered (OTR) in the state of Illinois (IL) Valid Regular Driver's License (DL) ANY of the following valid licenses/certifications: American Heart Association Basic Life Support (AHA BLS) American Heart Association Advanced Cardiac Life Support (AHA ACLS) American Heart Association Pediatric Advanced Life Support (AHA PALS)
Overview
Schedule
Monday – Friday, 8: 00a – 5:00p Plus one weekend per month and 2 holidays per year Typically sees 5–6 patients per day
Travel: Local travel required
Key Responsibilities
Perform patient assessments and develop/implement physician-approved treatment plans
Educate patients and families on therapeutic techniques and adaptive equipment
Design and fit orthotic, prosthetic, and assistive devices as needed
Conduct re-evaluations and adjust treatment plans as needed
Maintain clinical documentation and report patient progress to physicians
Coordinate outpatient referrals when in-home care isn't sufficient
Consult with and support other home health team members
Participate in required Agency meetings and ongoing professional development
